Job Description for Medical Assistant (Certified)

Certified medical assistants most often work in a doctor’s office, a nursing care center, or another medical facility such as a hospital, nursing home, or pharmacy. Their responsibilities include providing continuing patient care, working with doctors and nurses to meet the needs of patents, and performing administrative and clerical duties in an office environment. They greet and assist patients with paperwork, show patients to their exam rooms, and take patients’ vitals and drawing blood. Their typical work week usually includes a full-time schedule during regular business hours, although they may be hired for night shifts.

Certified medical assistants must have a high-school diploma or GED, as well as an associate’s degree in medical assisting or equivalent training/job experience (usually two to five years), and certification by the American Association of Medical Assistants. Certified medical assistants also usually possess at least a year of experience in the field or a related profession. They must also have CPR certification and experience working within HIPAA and other patient privacy laws. Other traits a medical assistant must possess include attention to detail, computer competency, and good interpersonal communication skills.

Free Medical Billing Training. Find out what Free will get you. You’ve heard the old saying – If it sounds to good to be true, it probably is. But there are some resources for learning about the medical billing process for free or reasonable cost.

A free introduction to the important concepts of medical billing and coding. Over an hour of video instruction. Covers many of the important subjects from the more comprehensive Medical Billing Fundamentals course ( Includes over $78 in FREE Material ).

There are some free “Mini Courses” online that are simply a very generalized document about medical billing and coding. It’s usually a document a few pages long or a short video presentation that gives a sample of their material. The hope is that you’ll be impressed enough to commit to purchasing the entire training program.

Be careful – I fell for this and spent several hundred dollars. It was an impressive looking website and slick marketing. Once I purchased all I received was a notebook of general information I could have easily found on the internet. Boy did I feel like a fool! And forget trying to get a refund.

Look out for Scams – Hold On to Your Credit Card

Also be wary of free medical billing training trial programs that require you to enter credit card information before viewing their “free” material. Supposedly if you decide not to take the complete course you have to call to cancel. I’m suspicious of anybody that wants a credit card up front. Once they get your credit card number it can be very difficult to get a refund or prevent them from charging you.

I’ve done this with other “free” offers before and found it almost impossible to cancel and get them to stop charging my credit card.

There are ways you can get free medical billing training. However the quality of the training may be lacking.

Get Real World Experience

Offer to work part time for a medical billing service for free – for a limited time of course. When I was anxious to learn medical billing, I offered to work for free. My thinking was that is a great way to get hands-on real world training.

I had a friend who started a medical billing business and was very busy. I talked to her about how she got started and asked for her advice. During our conversations I offered to work for free for a short time until I learned enough to start my own business.

Medical billing is the sort of work that can be done at home easily. Fortunately they didn’t take me up on the free part but did pay me for part time work from home. To me that was like getting paid to learn!

I started out working on insurance aging which involved calling insurance companies to follow up on unpaid claims and determine why they were rejected. I got progressively more experience and responsibilities until I felt comfortable marketing my own business.

There is a website called Quizlet which the worlds largest online student and teacher community. Their mission is to help students master the subjects they are learning. Quizlet has an almost overwhelming amount of medical billing and coding lessons, study guides, and tests from a wide variety of credible students and teachers.

Even better than free medical billing training is the thought of getting paid to learn. As mentioned above, any environment you can get into – even for pay that is less than the going rate – that allows you some experience in the field is a great help toward getting trained. This would be short term of course.

Consider opportunities at a medical billing services company, hospital or doctors office, or insurance company. Anything that gets you exposed to the field looks good on your resume. Remember your goal is getting experience – so a short term sacrifice in pay may be worth it to prove yourself and get good references.

One of the requirements I notice in medical billing job postings is experience in the field – probably more than training or credentials for medical billing. That’s why getting any experience you can is important. In this field there are many things that can’t be taught in a classroom environment.

There’s so much information from credible sources available on the internet on medical billing. You can educate yourself and find information on just about any topic. The challenge is being able to separate the credible and knowledgeable from the pretenders.

Even on this site I try to document my experiences, information, and other free medical billing training resources that may be helpful to others getting started in the medical billing and coding profession.

I found it can be difficult to find any in-depth information on particular topics. This takes a lot of time. There is a lot of online information on medical billing that seems to be almost entirely dedicated to marketing and not educating. These sites are just trying to sell something. Or on the opposite end of the spectrum having so much information that it’s overwhelming.

An example of this would be the HIPAA. There’s a tremendous amount of information about HIPAA available at CMS – The Centers for Medicare and Medicaid Services . But trying to determine what is relevant and applicable to a small practice is almost overwhelming as there are so many facets of HIPAA.

A lot of billing services are smart to educate themselves well on HIPAA (and other industry issues) and use this to educate, and market to, small practices. This is a great way to open doors and establish yourself as an expert.

American Association of Professional Coders ( AAPC ) is also has some great info on medical billing – although their main concentration is coding.

Medical Billing for Ambulatory Surgical Centers

An ambulatory surgical center (ASC) is a healthcare facility that is dedicated to providing medically necessary surgical services to a patient in the outpatient setting. ASCs are paid according to a unique set of regulations and standards under the Medicare program, under Medicaid, and under contractual agreements with private commercial health insurers. This article focuses on medical billing and medical coding in the ASC setting as it relates to Medicare, which is the template which other third-party payers follow.

Certified medical coders review a patient’s medical record and assign codes in the Healthcare Common Procedural Coding System (HCPCS) to describe the services performed. They also assign diagnosis codes from the International Classification of Diseases, 9th Edition, Clinical Modification (ICD-9-CM) to describe the reason for which the patient received surgery in an outpatient setting. They do not use ICD-9-CM codes from Volume 3 that describe procedures, because these codes are only used in the inpatient setting.

Professional medical billers assemble the codes on a CMS-1500 claim form, which is the universal standard for billing outpatient services, as established by the Health Insurance Portability and Accountability Act of 1997 (HIPAA).

How Ambulatory Surgical Centers are Paid

Since January 1, 2008, ASCs are paid for their services under Medicare Part B for covered services, as well as for ancillary services that are not considered a part of the surgical package assigned to a given HCPCS code. ASCs are reimbursed according to the Outpatient Payment System (OPPS) which provides a set payment for each surgical procedure and the incidental costs associated with them. Anything above and beyond the surgical package, as defined by the OPPS fee schedule, are billed to Medicare Part B on the same healthcare claim for separate payment.

The OPPS that governs ASC reimbursement is updated annually by the Centers for Medicare and Medicaid Services (CMS). The payments are determined by an weighted formula that considers average costs for each covered code listed in the HCPCS code books. Certified medical coders assign these codes based on relevant documentation after the services are performed. These payments are made to cover the technical costs incurred by providing pre-operative care, support in the operating suite, and recovery time spent in the facility. Professional services provided by surgeons and anesthesiologists are coded and billed by their own staffs, independent of the claims filed by an ASC.

OPPS payments for covered services include payment for nursing, technician, and support staff, as well as for the use of the ASC’s physical plant. Any laboratory tests related to the surgery that do not have to be performed in a certified laboratory, drugs, medical and surgical supplies, equipment and dressings, many implantable devices, radiology services performed in the operating suite, administrative costs, recordkeeping, and housekeeping are all considerations when CMS determines the appropriate fees for each applicable HCPCS code.

All these items are packaged together under a single HCPCS code for a single procedure, and each HCPCS code is assigned a set rate of payment to cover all these expenses. They final payment is the average cost of a given procedure. If a medically necessary surgical procedure is performed, no other considerations influence the final payment received.

Ambulatory Surgical Center Services

Not every surgical service that is assigned a HCPCS code is payable under the OPPS that governs ASC reimbursement. Medicare, as well as most other third-party payers, does not cover procedures provided in the ASC setting that will most probably result in an inpatient hospital admission for recovery. Nor does the ASC fee schedule contain procedures that are expected to be performed in a physician’s office. If performed in an ASC, the institution will not be paid for excluded procedures that CMS has determined should be performed in other settings.

Medical billing costs

Professional medical billers are conversant with the applicable regulations and contractual obligations that govern the reimbursement of services provided by ASCs. As with the rest of the healthcare industry, new covered procedures are added and deleted on a regular basis, as well as the services that are considered a part of the surgical package. As experts in the financial aspects of healthcare delivery, professional medical billers provide advice on which services can and cannot be provided by an ASC. They do this in conjunction with certified medical coders, who advise on the documentation requirements needed to report specific covered HCPCS codes to make sure procedures are being performed in the appropriate venue.

Everything provided to a patient in order to provide a surgical service in an ASC is assigned an appropriate code, and an associated charge, to report the actual cost implied by a HCPCS code under the OPPS. While the sum of the codes and charges do not affect final reimbursement, which is solely determined by the fee allotted to the primary HCPCS code, this information is tabulated by CMS to adjust the payment formulas, by either increasing or decreasing future reimbursement rates on the OPPS fee schedule.

Medical Coding and Medical Billing in an ASC

ASCs generally employ only trained and certified medical coders and medical billers. The legal compliance issues that govern ASC reimbursement are complex and nuanced. ASCs prefer to give these responsibilities to professionals who have graduated from an accredited program of study. As the complexity of healthcare administration increases, of which medical billing and medical coding are a part, formal education is an increasingly important requirement to entering this challenging and ever-expanding field. Universities, community colleges, and private schools have expanded their course offerings to meet this need in the industry. There are also online programs that provide training in medical coding and medical billing, that lead to eventual certification as a professional.

With a foundation in the basics of medical coding and billing, professional medical coders and medical billers are expected to understand not only the language of medicine, but also the language in which legal statues, regulations, and contracts are written. Medical billers and medical coders who practice their profession in the ASC setting specialize in a distinct set of codes that are reported in the billing environment different from that of other billers and coders. Published rules that govern ASC reimbursement are distinct from those that are followed in inpatient hospitals, in physician offices, in skilled nursing facilities (SNFs), and by Durable Medical Equipment (DME) suppliers.

In 2012, the Secretary of Health and Human Services is presenting a plan to Congress to implement value-based purchasing (VBP) when paying for ASC services through Medicare Part B. VBP may require ASCs to report additional codes in order to receive payment, and it may change the sequence and rationale that directs HCPCS coding in the ASC setting. Even if VBP does not become the standard in 2013, all indications are that it will be implemented at some time in the future. This is just one of the ongoing changes that affect ASC reimbursement, and it will not be the last.

Professional medical billers and certified medical coders are expected to be aware of the flux and requirements that influence ASC coding and billing. They are expected to adapt when the regulatory and contractual environment changes. They are able to do this because their education, training, and experience has prepared them to be able to do so. This is why an accredited program is a benefit for people looking to enter this dynamic field, and this is why employers look for that when they interview applicants to determine if they will add value to fiscal operations from the date-of-hire.

You asked how many insurance companies in Connecticut write medical malpractice insurance in other states, but not in Connecticut.

According to the Connecticut Insurance Department, there are 64 property and casualty insurance companies and six property and casualty reinsurance companies domiciled (based) in Connecticut. All of these companies, except for two, are authorized to write liability insurance (other than auto). (See Tables 1 and 2 below.) Medical malpractice insurance is one type of coverage under the liability insurance line of business. However, only one Connecticut-domiciled company, Connecticut Medical Insurance Company (CMIC), currently writes medical malpractice insurance. CMIC only writes the coverage in Connecticut.

There are 375 property and casualty insurance and reinsurance foreign companies (i.e., domiciled in a state other than Connecticut) doing business in Connecticut that are authorized to write liability insurance (other than auto). Only three of these foreign companies currently write medical malpractice insurance in Connecticut: Medical Protective, an Indiana-based company, ProSelect, a Massachusetts company, and Preferred Professional Insurance Company, a Nebraska company. However, the department does not know how many foreign companies write medical malpractice insurance in other states.

The department provided us with a list of all licensed insurance companies doing business in Connecticut. The list includes the company name, address, state of domicile, type of company (e.g., property and casualty, life and health), and line of business. While the insurance is not for everyone, a no medical exam life insurance policy has some key benefits. Making an educated decision about the appropriate policy starts with learning the advantages and disadvantages of the term life policy.

The benefit that most individuals are interested in with a no medical exam policy is the time required to obtain coverage. A policy that requires an exam will take as long as four to six weeks before the policy begins because consumers must first see a doctor and have reports sent to the insurance provider. The exams take time, require doctor s visits and might result in getting denied after the paperwork is finally completed. This type of policy is guaranteed because health conditions are not a factor in the decision to provide coverage.

Disadvantages of Coverage:
