Group Coverage Basics

Buying insurance for your small business (group coverage) has different rules than buying just for yourself or your family (individual coverage). Below, we outline the basics behind group coverage.

What Is Group Coverage?

Group medical coverage refers to a single policy issued to a group (typically a business with employees, although there are other kinds of groups that can get coverage) that covers all eligible employees and sometimes their dependents. Individual medical coverage, on the other hand, is a single policy issued to a single person or family.

The rules are quite different for group coverage versus individual coverage, in large part because the insurer s risk is calculated differently. With individual coverage, the insurer has historically based its premium rates (or denied coverage) on the detailed medical history of the person or family. (The Affordable Care Act brought important changes to the individual market, including eliminating the ability of insurers to deny coverage based on preexisting conditions.)

With groups such as small businesses, the insurer determines a premium price based on risk factors balanced over the entire group, using general information on members of the group, such as age or gender. Insurers are required by law to offer coverage to small groups.

Is Insurance Required?

While there is no law requiring small business owners to provide health insurance, the Affordable Care Act makes substantial changes that small business owners should be aware of when deciding whether to purchase insurance for their employees. If you do choose to offer coverage, there are regulations you will have to follow—the most important of which we explain on this site.

Though large companies may face penalties if they do not offer coverage under the Affordable Care Act, small businesses with fewer than 50 full-time-equivalent employees will not be penalized if they do not provide coverage. If you have at least 50 full-time-equivalent employees but none receive an individual premium tax credit or cost-sharing reductions (both based on income), there’s no penalty—whether or not you offer health insurance.

Is Your Business Eligible for Group Coverage?

Under federal law, small employers are guaranteed group coverage should they choose to purchase it, regardless of the employees’ health status. A “small employer” is defined as a business with 2 to 50 full-time employees. Owners are generally counted as employees, so sole proprietorships with one employee usually fall into this category, as do partnerships without any employees (by definition partnerships have two or more partners). Some states define the self-employed as groups of one and require insurers to guarantee issue them coverage in the small group market.

Who Is Eligible for Coverage?

The general rule is that if an employer offers group health coverage to any full-time employees, the employer must offer coverage to all full-time employees.

The employer has the option to offer coverage to part-time employees (defined as those working fewer than 30 hours per week). If the employer offers coverage to any part-time employees, all of them must be offered coverage.

These rules apply regardless of the medical condition of the employees. In other words, any eligible employee can t be denied coverage based on previous medical problems, known as preexisting conditions.

In addition, any dependents of eligible employees are generally eligible for coverage under a group plan. Dependents include spouses, children, and in some cases, unmarried domestic partners. Dependents cannot enroll for coverage unless the employee has enrolled. Under the Affordable Care Act, group insurance plans are required to extend coverage to adult dependents through age 26.

What Do Employers Have to Pay?

Some employers who decide to offer coverage choose to pay the full premium, while others require employees to pay a portion. When considering what portion of the premium to pay, employers should be aware that the Affordable Care Act offers small businesses tax credits to help offset the cost of insurance.

The small business healthcare tax credits have been available since the 2010 tax year, with significant changes implemented in 2014. To qualify for a tax credit of up to 50% of premium expenses for any two years, small business owners must pay at least half of employees’ healthcare premiums and have fewer than 25 full-time-equivalent employees who earn an average of $50,000 or less per year.

Waiting periods

You will have to serve a waiting period when you start a new private health insurance policy or increase your level of cover. A waiting period protects members of the fund by ensuring that individuals aren’t able to make a large claim shortly after joining and then cancelling their membership. This kind of behaviour would result in increased premiums for all members.

The government sets the maximum waiting periods that funds can impose for hospital treatment:

  • 12 months for pre-existing conditions,
  • 12 months for obstetrics (pregnancy),
  • two months for psychiatric care, rehabilitation or palliative care, even for a pre-existing condition,
  • two months in all other circumstances.

The waiting periods for general treatment (ancillary or extras) cover are set by individual health funds.

When purchasing health insurance make sure you are fully aware of any waiting period you may have to serve. There is usually no waiting period if you need hospital or medical treatment because of an accident that happens after you join the fund.

Pre-existing conditions

Under the Private Health Insurance Act 2007, a health fund may impose a 12 month waiting period on benefits for hospital treatment for a pre-existing condition.

A pre-existing condition is defined by law as any ailment, illness, or condition that you had signs or symptoms of during the 6 months before you joined a hospital table or upgraded to a higher hospital table. It is not necessary that you or your doctor knew what your condition was or that the condition had been diagnosed. A condition can still be classed as pre-existing even if you hadn’t seen your doctor about it before joining the hospital table or upgrading to a higher hospital table.

In forming an opinion about whether or not an illness was pre-existing, the health insurer appointed medical practitioner who makes the decision, must take into account information provided by your own doctor.

Your health fund will need time to advise you if your condition is pre-existing so be sure to check with your fund well before you go to hospital to make sure you are covered.

Even if you have a pre-existing condition, health funds must allow you to purchase any type of cover, at the same price as every other person. Once you have served any waiting periods, you will be entitled to claim.

Obstetrics (Pregnancy)

The maximum waiting period for obstetrics benefits is 12 months – if you are planning to become pregnant and wish to be covered, you will need to organise appropriate health insurance for both yourself and your newborn well in advance (before you become pregnant).

Entitlement to obstetrics benefits rests with the mother. She needs to have served the full waiting period before being admitted to hospital to be able to claim benefits.

Many less expensive hospital covers do not include obstetrics, or pay restricted benefits that only cover you for obstetrics as a private patient in a public hospital, not in a private hospital.

Health funds have different rules about how far in advance you need to make changes to your cover to insure your newborn baby. Some funds may require you to add your baby to your cover several months before his or her birth, so make sure you check with your fund as soon as possible.

    A pre-existing condition is one where signs or symptoms of your ailment, illness or condition, in the opinion of a medical practitioner appointed by GMHBA (not your own doctor), existed at any time during the six months preceding the day on which you purchased your hospital insurance or upgraded to a higher level of hospital cover and/or benefit entitlement.

    A special waiting period applies to obtain benefits for hospital treatment for new members who have pre-existing conditions. The waiting period also applies to existing members who have recently upgraded their level of hospital cover. If the ailment, illness or condition is considered pre-existing:

    New members must wait 12 months for any hospital benefits (other than psychiatric, rehabilitation and palliative care).

    Existing members (transferring or upgrading)

    Members transferring/upgrading to a higher hospital cover must wait 12 months to get the higher hospital benefits (other than psychiatric, rehabilitation and palliative care).

    Existing members (with less than 12 months membership)

    Existing members with less than 12 months membership in total across their old and new cover are entitled to the lower benefits on their old cover.

    A Benefit Limitation Periods (BLP) is a restriction on selected services for what GMHBA will pay for your hospital treatment for the first 24 months of cover. It begins the date you first join or upgrade your level of cover. Providing that you have served the relevant waiting period, if you need to go to hospital your benefit is restricted to public hospital benefits in a shared room. Once the 24 month BLP has been served you are eligible to claim in a Private Hospital as a private patient.

    E.g. After serving the two month waiting period for a procedure or service with a BLP, your benefits will be restricted for the following 22 months. After this period you will be entitled to claim this procedure or service in a private hospital.

    Silver Hospital cover provides the same single room coverage as Gold Hospital, but co-payments of $100 per day up to a maximum of $700 per admission apply. Co-payments are not the same as hospital excess. Please note: Some private hospitals only have single rooms and co-payments will apply in a public and private hospital.

Or, in two 1 liter bottles: Juice of 3 lemons, divided equally between the two bottles (about 85ccm or 3 oz per bottle) An equal quantity of grade B or C maple syrup in each bottle (about 85 ccm or 3 oz per bottle) A pinch of cayenne in each bottle Spring or purified water (fill bottles to the top) Mix all the ingredients by thoroughly shaking; then drink throughout the day. Use fresh lemons or limes only, never canned or frozen lemon juice. Use organic and vine ripened when possible. Also, mix your lemonade fresh just before drinking. Don’t mix it up in the morning for the whole day. You can, however, squeeze your lemons in the morning and measure out the 2 tablespoons when needed.

    * YL Clinic says you can use Agave instead of the maple syrup. Tom Woloshyn (who studied with Stanley Burroughs himself) disagrees. Clinic says if you have diabetes or hypoglycemia, use molasses. If you feel weak or have a sugar imbalance you can add a scoop of Power Meal. Or you can try Master Amino Acid Profile (MAP) for protein without any residuals or strain on the body.

    Drink an oral salt water enema upon arising. To do this, add 2 level teaspoons of uniodized sea salt to a quart of lukewarm water (the one-quart juice bottles in which most organic juices come work very well). Shake well, then drink the entire quart. It’s also good to massage the colon as well. Make sure you use uniodized sea salt; regular or iodized salt will not have the same beneficial effect. This oral enema will flush out your entire digestive tract and colon from top to bottom, usually within an hour, prompting you to eliminate several times, clearing out the plaque and debris from the walls, and the parasites that have been living there.

    Lemonade Drink:
    Drink the lemonade mix every 1 to 2 hours. Take no other food, but do be sure to drink plenty of purified water in addition to the lemonade drink. Drink as much of this lemonade as you want, but make sure that you drink at least twelve 1/4 liter (8 oz) glasses. The lemonade contains all the vitamins and minerals you need. This can be combined with supplements for colon cleansing such as Bentonite or Psyllium Seeds. You don’t have to start your fast on the morning of the first day. You can begin later in the day, even if you’ve already eaten. Once you begin, however, eat nothing more while you’re on the fast. It’s also a good idea to read Stanley Burrough’s book, The Master Cleanser. Note: Diabetics, refer to the special instructions on page 19 of The Master Cleanser.

    Note: Bentonite is the only product known to remove plaque from the walls of the intestine and colon. The plaque in your stool will look like egg shells on the outside of the bentonite “gel.” Bentonite works most effectively when the colon is empty of food. It grabs the plaque which has been loosened by the lemon juice and the resting/fasting process. The salt water enema pushes out the bentonite gel and the plaque and debris that is stuck to it and in it. The salt water further cleanses the walls as it passes through, resulting in shiny clean walls.

    Herbal Laxative:
    Each evening you can drink an herbal laxative tea to help with elimination, do this right before bed time.

    Burroughs recommends a minimum of 10 days on this. You can safely do 40 days or more.
    First Day: Start with 4 oz. [1/8 liter] fresh squeezed orange juice mixed with 4 oz. water. If it goes well, drink several more 8 oz. [1/4 liter] glasses of fresh orange juice during the day. Sip slowly. Dilute with water if needed.
    Second Day: Drink several 8 oz. [1/4 liter] glasses of orange juice during the day — with extra water, if needed.

    In the evening make a vegetable broth (no canned soup). Use seasonal leafy and root vegetables such as: beets and beet tops, turnips and turnip greens, kale, carrots, onions, parsley, celery, potatoes, okra, one or two inds of legumes, squash, beans, a little salt, cayenne pepper and dehydrated vegetables or veg. powder may be added for flavor (no MSG or hydrolyzed protein).
    Cook lightly. Drink the broth, eating only a few bites of the vegetables.
    Third Day: Orange juice in the morning. At noon have some more soup with some of the vegetables. No meat, fish, eggs, bread, pastries, tea, milk, or coffee. For Dinner, have the vegetables in the soup.
    Fourth Day: Orange juice or lemon and maple syrup in the morning. Fruits, vegetables, seeds, nuts for lunch. Salad or fruit for dinner.
    Fifth Day: Eat normally but no junk food, dairy, tea, coffee, white flour or white rice, heavy animal proteins. If, after eating is resumed, distress or gas occurs, go back to the lemonade diet for a few days until the system is ready for food.

    This CAHIM accredited degree program is designed to provide students with the technical expertise in management of health information contained both in paper and electronic formats. The student will obtain knowledge and skills to perform job functions in medical records, medical coding, medical billing and other information-based areas in both the hospital and outpatient settings. Graduates of the program will be able to provide reliable and valid information that drives the health care industry.

    This program provides students with the technical expertise in health data collection, analysis, monitoring, maintenance, and reporting activities in compliance with established legal, ethical, regulatory and professional standards. Course content will include both paper and electronic information management concepts and technologies, in addition to ethical and medico-legal aspects, computer information technology, biomedical sciences, health record science, statistics and data literacy, medical coding, clinical classification systems, reimbursement methodologies, quality assessment, health care delivery systems, indexing, performance improvement and professional practice experience.

    Program Accreditation
    The Health Information Technology AS degree program is accredited by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM). This accreditation confirms that the program has voluntarily undergone a rigorous review process and has been determined to meet or exceed the Standards set by the Board of Directors. Graduates are eligible to apply and take the national certification exam for Registered Health Information Administrator (RHIA) or Registered Health Information Technician (RHIT).

    The program is offered at the Lake Worth campus. However, we are expected to move to the new Loxahatchee Groves campus prior to the Spring 2017-2 semester.

    Employment Opportunities
    The roles commonly filled by a registered health information technician (RHIT) include: cancer (or other disease) registrar, clinical coder/compliance auditor/vocabulary specialist, clinical data collection and reporting specialist, data integrity specialist, document imaging coordinator, information access/disclosure specialist, quality improvement specialist, reimbursement specialist/financial services liaison, and instructor/trainer.

    Upon completion students are eligible to sit for the Registered Health Information Technician (RHIT) exam provided by the American Health Information Management Association.

    The Body Clock: Eating Schedule Is Equally Important As Sleep Cycle For Maintaining Rhythm

    Planning out healthy meals throughout the day is a luxury some of us cannot afford. More and more people are skipping meals, eating outside the home, on the go, later in the day, and more irregularly due to more shift work and social jetlag — when someone has different sleeping patterns on weekends and weekdays. It would be nice to have a flexible meal schedule and healthy food on hand, but what’s more important in the long run: what we eat or when we eat it?

    In two separate papers published in the Proceedings of the Nutrition Society. researchers from King’s College London, Newcastle University, University of Surrey, and Nestlé Research Centre examine how our metabolic processes can alter or influence our internal clock when the timing, size, and quality of our meals start to go haywire.

    “Whilst we have a much better understanding today of what we should be eating, we are still left with the question as to which meal should provide us with the most energy,” said Dr. Gerda Pot, Visiting Lecturer in the Diabetes and Nutritional Sciences Division at King’s College London, in a statement .

    Shift Work

    Pot and her colleagues combined data from a number of dietary studies with global differences in eating habits. Studies continually show how shift workers face an increased risk for Type 2 diabetes, heart disease, and cancer. Alternating between irregular and around-the-clock shifts cause a disruption in our circadian rhythm, and appetite, digestion, and metabolic processes are highly dependent on its 24-hour clock.

    Social Jetlag

    The researchers estimate that social jetlag affects more than 80 percent of people in central Europe, especially those living in urban areas. By staying up late and sleeping in on weekends, then going to bed early to wake up early during the week, we set up a mismatch between our circadian rhythm and our social sleep schedule. Over time, this imbalance causes metabolic changes that lead to obesity, heart disease, and cancer.

    When We Should Eat

    Different eating patterns from around the world depend on personal preference and local culture. For example, the French place their importance on pleasurable and social eating, which is why many people in France and the Mediterranean region consider lunch the most important meal of the day. Food choices in central England, on the other hand, reflect individual preferences and convenience. This often translates into skipping meals, calorie-dense snack foods, and a higher consumption of ready-made and fast-food meals.

    Currently in both the United States and the United Kingdom, energy intake from food consumption tends to increase throughout the day. Breakfast provides the lowest amount of energy while dinner provides the highest. This same shift toward smaller breakfasts and bigger dinners has started in France in recent years as work patterns have changed. So are the people of France at risk for an obesity epidemic similar to the one seen in the U.S.?

    Is Breakfast King?

    In a recently concluded study. researchers from Umea University asked 889 16-year-old students about their breakfast eating habits, propensity to skip meals, and how important sweetness was to their enjoyment of food. Nine percent of the students practiced poor breakfast habits. When the research team caught back up with the participants at the age of 43, they found that people who skipped breakfast as kids were 68 percent more likely to have metabolic syndrome.

    “There seems to be some truth in the saying ‘Eat breakfast like a king, lunch like a prince, and dinner like a pauper,’ however, this warrants further investigation,” Pot added. “Although the evidence suggests that eating more calories later in the evening is associated with obesity, we are still far from understanding whether our energy intake should be distributed equally across the day or whether breakfast should contribute the greatest proportion of energy, followed by lunch and dinner.”

    When We Should Work Out

    Sleep and meals aren’t the only things whose timing influences our health — the time of the day we exercise also plays a role. Just like our dieting habits, when we work helps determine how effective exercise can be. However, there seems to be no clear winner between working out in the morning or at night. Both have their benefits: Working out in the evening is best for boosting muscle strength and size, while exercising in the morning helps with losing weight, improving sleeping, and reducing stress.

    Career Technical Education Standards for California Public Schools

    Education Code Section 51226 provides legal authority to develop the CTE standards and framework. This legislation requires the development and adoption of CTE standards that incorporate the integration of career technical and academic education no later than June 1, 2005. The original CTE Model Curriculum Standards were adopted by the State Board of Education (SBE) on May 11, 2005. The standards, written for grades seven through twelve, specify learning goals in 58 career pathways organized around 15 industry sectors.

    The California State Plan for Career Technical Education, A BRIDGE TO THE FUTURE 2008 2012, approved by the State Board of Education in May 2008, provides guidance for California s CTE programs in California. The State Plan states, CTE programs are dynamic; curricula need to stay current with rapid changes in the workplace, requiring ongoing updates and learning on the part of CTE faculty. The adoption of the English Language Arts and Mathematics Common Core State Standards furthered the need to revise and align the CTE Standards with this new academic core.

    Commencing in May 2011, 117 individuals representing secondary and post-secondary education and business and industry met to review the 2005 standards and make recommendations for improvement. Following the May 2011 meeting, industry sector meetings were held to develop the revised CTE standards based on the recommendations. Subsequently, the revised CTE Standards were share with the general public for public review, comment, and suggestions. Once the CTE Standards were revised, academic and CTE teachers collaborated on the alignment with the Common Core State Standards, Next Generation Science Core Ideas, and the History/Social Science Standards. The newly revised CTE Model Curriculum Standards designed to prepare students to be both Career and College ready were adopted by the SBE on January 16, 2013.

Approved by the Pennsylvania State Board of Nursing, the curriculum reflects a 16-month nursing program (60 weeks of instruction with breaks throughout), with a requirement of four (4) pre-requisite college credit* courses completed prior to entry, for a potential total of 20 months leading to a diploma in nursing.

    To be considered for January 2018, admission, all pre-requisites need to be successfully completed (with a C (2.0) or better) by a date to be determined. An official transcript showing proof of completion must be received by the Washington Health System School of Nursing prior to the start of orientation to begin enrollment with the cohort of students in May 2017.

    *Pre-requisite college credits include: Human Anatomy (or Anatomy and Physiology I) with lab, Human Physiology (or Anatomy and Physiology II) with lab, English Composition, and Developmental Psychology. NOTE: All science credits (Anatomy and Physiology) must have been completed within the past five years, as well from the same institution.

    The admission procedure includes the ATI TEAS examination with a minimum Academic Preparedness Level of Proficiency with a score of greater than or equal to 65% being recommended. If the ATI TEAS is taken a second time, the higher score will be considered. Applications are also currently being accepted for the Class of 2019 (January, 2018, start date).     This year we refined our organizational structure and expanded collaborations to support achievement of the Center of Innovation (COIN) program goals. We also have supported an emerging area of research concentrated on improving the quality, safety, and value of clinical therapeutics in the VA and beyond.

    CHERP leaders consistently respond to new opportunities to communicate the value of our research to VA leaders charged with ensuring the quality and equity of health care for millions of Veterans. CHERP Investigators are at the forefront of efforts to evaluate the impact of VA’s implementation of the patient-aligned care team model of care and efforts to translate VA research findings into clinical practice.

    About Our History

    The VA Center for Health Equity Research and Promotion (CHERP) was established in 2001 by HSR D as a national Center of Excellence focused on detecting, understanding, and reducing disparities in health and health care in vulnerable populations. This strong collaboration between the VA Pittsburgh Healthcare System (VAPHS) and Philadelphia VA Medical Center (PVAMC) in VISN4 and their respective academic affiliates (University of Pittsburgh and University of Pennsylvania) was built upon the collective and complementary strengths of the investigators at both sites to target an area of investigation of highest priority to the VA Healthcare System and the nation as a whole. Built upon a track record of scholarship, cross-center research collaborations, development and retention of highly accomplished investigators, service, and well-established partnerships, CHERP became a Center of Innovation (COIN) that has measurable impact on the quality and equity of Veteran health care.

    About Our Investigators

    Over the past decade, CHERP investigators conducted innovative, high impact health services research; advanced the conceptual underpinnings of health equity research to facilitate the work of others; partnered with operations and policy leaders on research and quality improvement initiatives; promoted the implementation and dissemination of findings to improve the clinical care and health outcomes of Veterans; and supported the development of junior investigators who have become leaders in this field. CHERP provides infrastructure and support for approximately 42 core investigators engaged in health services research at the VA Pittsburgh Medical Center and the Philadelphia VA Medical Center.     Long-Term Care is Different From Traditional Medical Care
    Someone with a prolonged physical illness, a disability or a cognitive impairment such as Alzheimer s disease often needs long-term care. Long-term care services may include help with daily activities, home health care, respite care, hospice care, adult day care, care in a nursing home or care in an assisted living facility.

    Long-Term Care Can be Expensive
    The cost depends on the amount and type of care you need and where you get it. In 2001, the national average cost of nursing home care was $56,000 per year, assisted living facilities reported $22,476 per year and home care costs ranged from $12,000 to $16,000 per year.

    You Have Options When Paying for Long-Term Care
    People pay for long-term care in a variety of ways. These include using personal resources, long-term care insurance and Medicaid for those who qualify. Medicare, Medicare supplement insurance and health insurance you may have at work usually will not pay for long-term care. Long-term care insurance will pay for some or all of your long-term care.

    Decide Whether Long-Term Care Insurance is for You
    Whether you should buy a long-term care insurance policy will depend on your age, health status overall retirement goals, income and assets. For instance, if your only source of income is a Social Security benefit or Supplemental Security Income (SSI), you probably should not buy long-term care insurance since you may not be able to afford the premium. On the other hand, if you have a large amount of assets but do not want to use them to pay for long-term care, you may want to buy a long-term care insurance policy. Many people buy a policy because they want to stay independent of government aid or the help of family. They don t want to burden anyone with having to care for them. However, you should not buy a policy if you can t afford the premium or are not sure you can pay the premium for the rest of your life.

    Pre-Existing Condition Limitations
    A long-term care insurance policy usually defines a pre-existing condition as one for which you received medical advice or treatment or had symptoms within a certain period before you applied for the policy. Some companies look further back in time than others. Many companies will sell a policy to someone with a pre-existing condition. However, the company may not pay benefits for long-term care related to that condition for a period after the policy goes into effect, usually six months.     Contact several companies and agents before you buy a long-term care policy. Be sure to compare benefits, the types of facilities covered, limits on your coverage, what is not covered and the premium. Policies from different insurance companies often have the same coverage and benefits but may not cost the same. Be sure to ask companies about their rate increase history and whether they have increased the rates on the long-term care insurance policies.

    Don t be Misled by Advertising
    Most celebrity endorsers are professional actors paid to advertise, not insurance experts. It is also important to note that Medicare does not endorse or sell long-term care insurance policies, so be wary of advertising that suggests Medicare is involved. Do not trust cards you get in the mail that look like official government documents until you check with the government agency identified on the card.

    The Health Information Management online associate degree program is accredited by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM). According to the Bureau of Labor Statistics, health information management jobs are expected to increase by 15% from 2014-2024. The median income is $37,110, (2015 statistic), but additional credentials do add to the marketability of professionals.

    The HIM program was established in 1998 as a traditional face-to-face program. In 2007-2008, the HIM program transitioned to an “online only” program to meet the needs of working students and professionals. The exception to online courses are the requirement for clinical courses completed onsite at a facility (HITT 2261 for the associate degree and HITT 1167 for Medical Coding Specialist certificate).

    Many students admitted into the program have already completed one degree, and some have bachelors or masters degrees. The reason they pursue the associate degree training is to be eligible for the Registered Health Information Technician (RHIT) upon graduation. The RHIT credential, and the RHIA credential for the bachelor’s degree, continue to be a preferred credential that healthcare employers seek when hiring Health Information Management candidates.

    Students must be self-motivated, able to comprehend information in written form, and have the ability to communicate effectively both in written and verbal forms. Students should also be able to research websites, upload and download documents, post discussions, and access email. The courses are not self-paced, but have scheduled weekly due dates for assignments, discussions, and tests. Students should expect to spend 6 9 hours per 3 hour credit course to complete assignments, readings, and tests. Instructors are available to answer questions online or by phone related to course topics.

    Skills Needed for Health Information Profession:

    ability to analyze patient data for payment or registry information,
    able to sit for long periods to complete work at work station
    research and review patient information for regulatory compliance,
    code and categorize patient information using standard coding systems.
    communicate effectively with patients, physicians, and other customers
    ability to work under pressure to meet deadlines
    keyboarding skills and ability to use software systems, patient portals, etc.

    Students may apply to the program in the semester they are completing the prerequisites. Final grades will be verified before admittance. Students are accepted prior to each semester (Summer, Fall, and Spring). Most of the students in the programs are considered part-time; taking 2 to 3 classes each semester since most have part-time or full-time jobs. There are approximately 70-80 students taking HIM classes or prerequisite courses to gain admission to the program.

    Clinical Requirement Onsite:

    Students that live outside the local Midland area are able to locate clinical sites in their area with the assistance of the HIM program. More information and directions are given to students upon acceptance into the program (courses related to associate degree and Medical Coding Specialist certificate). Prior to the clinical course, students are required to meet certain facility requirements to attend clinical practice. These include a background check, drug screen, immunizations, and other requirements based of the facility. More information provided prior to clinicals.

    The University of Hartford is a fully accredited, private, co-educational institution with the main campus located on approximately 350 wooded acres in West Hartford, Connecticut.     Medicare vs. Medicare Advantage: How to Choose

    Decoding health insurance options can be daunting for people age 65 and over. Those who have paid 10 years of Social Security taxes qualify for Medicare at age 65. They’re automatically signed up if they’re receiving Social Security payments, unless they take steps to opt out. Standard Medicare comes in two parts: A and B. Part A covers a portion of hospitalization expenses. and Part B applies to doctor bills and other medical expenses, such as lab tests and some preventive screenings .

    But some seniors may find better value in Medicare Part C, or Medicare Advantage. Such plans are run by private insurance companies but regulated by the government, and must offer coverage that’s comparable to original Medicare parts A and B. Most also include prescription drug coverage, which is an optional add-on called Part D for seniors who keep original Medicare.

    Some Medicare Advantage plans cost nothing more per month than original Medicare, while others come with a higher monthly premium. (Any additional premium is added to what you would otherwise pay for Medicare Part B – about $105 per month in 2013, which for most people is automatically deducted from their Social Security checks.) Medicare Advantage plans are similar to private insurance that self-employed individuals buy on the open market, in that they have different monthly premiums, copays, coinsurance and out-of-pocket limits. The trade-off for a lower premium (or none) could be higher copays or coinsurance.

    Whether or not a Medicare Advantage plan costs more, it could be better or worse for you than original Medicare. Consumers have to carefully review the details of each plan and make a clear-eyed appraisal of their circumstances, including their health, budget and tolerance for financial risk.

    You May Want to Get Medicare Advantage Instead of Original Medicare If.

    • You take prescription drugs. As stated, original Medicare doesn’t cover prescriptions unless you pay extra to add Part D coverage. (The average monthly cost of Part D was $40 in 2013.) By contrast, about 82 percent of Medicare Advantage plans include drug coverage, according to the Kaiser Family Foundation, a nonprofit, nonpartisan research institute. In some cases your monthly premium will exceed the amount you’d pay for part D. The federal government. and brokers like have online tools where you can check how much you’ll pay for the drugs you need. (U.S. News has a revenue-generating agreement with eHealthInsurance, which owns
    • You want a cap on your out-of-pocket health spending. Original Medicare has no out-of-pocket maximum. You keep paying a portion of the cost of services as you use them. Medicare Advantage plans, by law, have an out-of-pocket maximum of no more than $6,700 per year. Once you hit that limit, the plan pays for all covered expenses. In 2013, 48 percent of Medicare Advantage plans had a $3,400 cap or less, according to the Kaiser Foundation. Keep in mind you might not hit that cap: Average out-of-pocket Medicare spending was $2,536 in 2006, or about $3,000 in today’s dollars, according to a recent analysis by Kaiser.
    • You want an alternative to enhancing your Medicare coverage with private “Medigap” (Medicare Supplement) insurance. Medigap plans cover or help cover certain deductibles, coinsurance and out-of-pocket costs of original Medicare. Some Medicare Advantage plans, but certainly not all, will be more cost-effective than adding Medigap coverage to original Medicare. Scrutinize the plan details if this is your reason for considering Medicare Advantage.
    • You want an alternative to the 20-percent coinsurance charged by original Medicare for most services. Medicare Advantage plans structure costs differently and may offer a lower (or higher) coinsurance, or may charge copays instead – which are fixed costs you pay for services. A trade-off could be a higher monthly premium.
    • You want coverage for vision, dental, assisted living facilities or nursing home care. Original Medicare doesn’t cover these services. Certain Medicare Advantage plans do.
    • You want the broadest possible choice in doctors and other medical providers. More doctors and other providers accept original Medicare than private Medicare Advantage insurance. Private plans tend to be restricted to a home network, like an HMO doctor network. If you travel frequently, you may want to consider staying with original Medicare for this reason.
    • You can’t afford or don’t want to pay additional monthly premiums. Medicare Advantage plans, like other private health plans, typically charge premiums. The recent average was about $30 a month for an HMO and $32 to $63 for a PPO, according to the Kaiser Foundation. Again, these premiums don’t include your Part B premium – about $105 per month in 2013 (or higher if you are high-income).
    • You want maximum flexibility when seeking medical specialists. Under original Medicare, you don’t need prior authorization from a primary care doctor to see a specialist, whereas Medicare Advantage plans that are designated HMOs could require you to see a primary care doctor first. PPO plans would allow you to see a specialist without a referral, but seeing an out-of-network doctor or specialist would cost you more. Most Medicare Advantage plans are either HMOs or PPOs.
    • You’re still employed and covered by your employer. You might end up paying an unnecessary premium for Medicare Advantage or could lose your employer-provided coverage. Check with your human resources department and the Social Security Administration for specifics.
    • You have employer-sponsored retiree health benefits that supplement original Medicare. These benefits wouldn’t help with Medicare Advantage, so check with your human resources department before signing on a Medicare Advantage plan.
    • You qualify for Medicaid or a Medicare Savings Program. Low-income Medicare beneficiaries have other options and should contact their state Medicaid office.

    Singapore ranked best place to live in Asia for expats: Mercer s survey

    VIENNA: Singapore was ranked as the best place to live in Asia for expatriates and 25th best in the world in the 19th Quality of Living survey by consulting firm Mercer’s released on Tuesday (Mar 14).

    Vienna, Austria’s grand capital on the Danube river, was ranked first for the eighth year in a row, while Baghdad is again considered the worst place to live.

    Two Japanese cities, Tokyo and Kobe, were the next highest-ranked Asian cities after Singapore at 47th and 50th globally. Hong Kong came in at 71st place, Kuala Lumpur 86th, Shanghai 102nd, Bangkok 131st, Manila 135th and Jakarta 143rd.

    The survey also includes a city infrastructure ranking that assesses each city’s supply of electricity, drinking water, telephone and mail services, and public transportation as well as traffic congestion and the range of international flights available from local airports. Singapore topped the list ahead of Frankfurt and Munich, while Baghdad and Port au Prince ranked last.

    City infrastructure plays an important role when multinationals decide where to establish locations abroad and send expatriate workers, Mercer’s said in a press release.

    “Easy access to transportation, reliable electricity, and drinkable water are all important considerations when determining hardship allowances based on differences between a given assignee’s home and host locations,” it said.

    The survey of 231 cities helps companies and organisations determine compensation and hardship allowances for international staff. It uses dozens of criteria such as political stability, health care, education, crime, recreation and transport, according to its website.

    Global centres London, Paris, Tokyo and New York City did not make the top 30, lagging behind most big German, Scandinavian, Canadian, New Zealand and Australian cities.

    San Francisco, placed 29th, was the highest entry in the US. Top of the list in Africa was South Africa’s Durban at 87.

    Physician Assistant Overview


    Physician assistants diagnose illnesses, develop and carry out treatment plans, assist in surgeries, perform procedures and guide patients. Jeffrey Katz, president of the American Academy of Physician Assistants (AAPA), says, “I diagnose and treat patients, illnesses and diseases and counsel them on their path to wellness.”

    Their work is very similar to that of a general internist or doctor, but they are required by law to practice under the supervision of a licensed physician or surgeon. Often, this supervision is more like collaboration, but there are certain archaic regulations that make life for physician assistants – and their patients – difficult. “To give a real-life example of these arbitrary regulations, in my practice, I can write a patient a prescription for morphine,” Katz explains. “However by law, I cannot prescribe my diabetic patients diabetic shoes.”

    “Often the stress comes in when PAs are not able to practice to the full extent of their training, education and ability,” Katz says.

    But the profession is filled with rewards that come from helping and treating patients. A 2015 AAPA study found that more than 96 percent would recommend their physician assistant career to others. Katz has worked in the same family practice in Taylorsville, North Carolina, for more than 20 years and has seen generations of families. “It is really cool to see the children of children. … I don’t think there’s any better gift,” he says.

    The U.S. is currently facing a shortage of health care professionals, and physician assistants – who are a cost-effective alternative to general internists – are needed to fill the void. From 2014 to 2024, the BLS projects that this field will grow at a rate of 30 percent, which will shake out into 28,700 new jobs for physician assistants.

    It’s not a very good time to be a prisoner in the United States.

    Incarceration is not meant to be fun, of course. But a combination of strict sentencing guidelines, budget shortfalls and a punitive philosophy of corrections has made today’s prisons much more unpleasant–and much less likely to rehabilitate their inhabitants–than in the past, many researchers say.

    What is the role for psychologists? First and foremost, they are providing mental health services to the prison population, which has rates of mental illness at least three times the national average.

    More broadly, they are contributing a growing body of scientific evidence to political and philosophical discussions about the purpose of imprisonment, says Craig Haney, PhD, a psychologist at the University of California, Santa Cruz.

    “Psychology as a discipline now has a tremendous amount of information about the origins of criminal behavior,” says Haney. “I think that it is important for psychologists to bring that information to bear in the debate on what kind of crime control policies we, as a society, should follow.”

    Until the mid-1970s, rehabilitation was a key part of U.S. prison policy. Prisoners were encouraged to develop occupational skills and to resolve psychological problems–such as substance abuse or aggression–that might interfere with their reintegration into society. Indeed, many inmates received court sentences that mandated treatment for such problems.

    Since then, however, rehabilitation has taken a back seat to a “get tough on crime” approach that sees punishment as prison’s main function, says Haney. The approach has created explosive growth in the prison population, while having at most a modest effect on crime rates.

    As a result, the United States now has more than 2 million people in prisons or jails–the equivalent of one in every 142 U.S. residents–and another four to five million people on probation or parole. A higher percentage of the population is involved in the criminal justice system in the United States than in any other developed country.

    Many inmates have serious mental illnesses. Starting in the late 1950s and 1960s, new psychotropic drugs and the community health movement dramatically reduced the number of people in state mental hospitals. But in the 1980s, many of the mentally ill who had left mental institutions in the previous two decades began entering the criminal justice system.

    Today, somewhere between 15 and 20 percent of people in prison are mentally ill, according to U.S. Department of Justice estimates.

    “Prisons have really become, in many ways, the de facto mental health hospitals,” says former prison psychologist Thomas Fagan, PhD. “But prisons weren’t built to deal with mentally ill people; they were built to deal with criminals doing time.”

    The plight of the mentally ill in prisons was virtually ignored for many years, but in the past decade many prison systems have realized–sometimes with prodding from the courts–that providing mental health care is a necessity, not a luxury, says Fagan.

    In many prison systems, psychologists are the primary mental health care providers, with psychiatrists contracted on a part-time basis. Psychologists provide services ranging from screening new inmates for mental illness to providing group therapy and crisis counseling.

    They also provide rehabilitative services that are useful even for prisoners without serious mental illnesses, says Fagan. For example, a psychologist might develop special programs for substance abusers or help prisoners prepare for the transition back to the community.

    But they often struggle to implement such programs while keeping up with their regular prison caseloads. “We’re focused so much on the basic mental health services that there’s not enough time or emphasis to devote to rehabilitative services,” says Robert Morgan, PhD, a psychologist at Texas Tech University who has worked in federal and state prisons and studies treatment methods for inmates.

    Part of the problem is limited resources, says Morgan: There simply aren’t enough mental health professionals in most prisons. Haney agrees: “Many psychologists in the criminal justice system have enormous caseloads; they’re struggling not to be overwhelmed by the tide.”

    Another constraint is the basic philosophical difference between psychology, which is rehabilitative at heart, and corrections, which is currently punishment-oriented.

    “Right now there’s such a focus on punishment–most criminal justice or correctional systems are punitive in nature–that it’s hard to develop effective rehabilitative programs,” says Morgan.

    To help shift the focus from punishment to rehabilitation, psychologists are doing research on the causes of crime and the psychological effects of incarceration.

    In the 1970s, when major changes were being made to the U.S. prison system, psychologists had little hard data to contribute.

    But in the past 25 years, says Haney, they have generated a massive literature documenting the importance of child abuse, poverty, early exposure to substance abuse and other risk factors for criminal behavior. The findings suggest that individual-centered approaches to crime prevention need to be complemented by community-based approaches.

    Researchers have also found that the pessimistic “nothing works” attitude toward rehabilitation that helped justify punitive prison policies in the 1970s was overstated. When properly implemented, work programs, education and psychotherapy can ease prisoners’ transitions to the free world, says Haney.

    Finally, researchers have demonstrated the power of the prison environment to shape behavior, often to the detriment of both prisoners and prison workers.

    The Stanford Prison Experiment, which Haney co-authored in 1973 with Stanford University psychologist and APA Past-president Philip G. Zimbardo, PhD, is one example. It showed that psychologically healthy individuals could become sadistic or depressed when placed in a prison-like environment.

    More recently, Haney has been studying so-called “supermax” prisons–high-security units in which prisoners spend as many as 23 hours per day in solitary confinement for years at a time.

    Haney’s research has shown that many prisoners in supermax units experience extremely high levels of anxiety and other negative emotions. When released–often without any “decompression” period in lower-security facilities–they have few of the social or occupational skills necessary to succeed in the outside world.

    Nonetheless, supermax facilities have become increasingly common over the past five to ten years.

    “This is what prison systems do under emergency circumstances–they move to punitive social control mechanisms,” explains Haney. “[But] it’s a very short-term solution, and one that may do more long-term damage both to the system and to the individuals than it solves.”

    My father has been doing rehab at this facility recently. I have found the staff to be caring, kind and available to answer any of my questions or concerns. The dietary staff has been eager to work with me on my father’s likes and dislikes and I found the CNA’s to be knowledgeable and attentive to my dad’s condition and needs. The facility is due for a renovation, but the facility is clean and the hands on care is good.

    My friend is now there. She had this beautiful quilt, hand made which was taken to the so called storage when admitted to hospital. When she was returned to the facility the beautiful quilt was never returned, some of her clothes were not returned either. I went to office and spoke with the girl at the office, spoke with the lady which cleaned the room of her belonging, I was taken to a rack where clothes of patients are being held. Names on clothing do not help clothes are being given to other people. The rugs are dirty and smelly they are old and should be replaced. Beds are not equipped properly, patients can easily get off when they shouldn’t. This is because the beds do not have the railings it should have. They have alarms on the beds and mats on the floor and when a patient that shouldn’t get up gets up the alarms goes off and it takes forever before a CNA or nurse appears. There is a lack of communication between nurses, doctors and family members. I asked a question regarding blood testing. The response I received from a nurse who couldn’t read the lab results very well were astonishing. I could go on but I think that gives you an idea of some of the things which should be looked at. It is an abuse if personal property is taken and not returned. They need more help for all the patients they have. Some of the CNA Nurses are awesome but so short handed to do the work they would like to do.

    This facility does not have enough CNA staff to meet the needs of the resident. During meal times we have to wait over an hour if our family member needs to be changed or moved. If we want to put our family member in a wheel chair, we can not do it when close to meal time or if we return during meal time our family member has to sit in the wheel chair until all the meals are passed out and those that can not feed themselves are fed. Also the one CNA assigned to your room may go on break right after meal service which will delay any further help, they don’t offer to get a floater, if there is one, to help. My assestment is they are low staffed even though they say they meet the state requirements, the staff they have are doing the job of 2 people and therefore the family member suffers while waiting for help.

    This place is awful. I would never send anyone here (unless I wanted to kill them ha) The staff is incompetent and don’t like their jobs, and how can I blame them for that. I’m told the DON and this woman Linda something or another are THE WORST. The staff hate them and so they don’t take care of their patients. Working here is a career killer, would NEVER step foot in the place. It smells and might as well be shut down because really are they helping people. probably as much as a dog chasing its tail.
